ABSTRACT
The article discusses the use of weaning protocols and specialized weaning units for patients who fail to wean from mechanical ventilation. Discontinuation of ventilation is estimated to take up to 40% of the total duration of ventilatory support and around 3-6% of patients admitted to the intensive care unit require a prolonged course of mechanical ventilation. There is now a substantial body of evidence confirming that the application of non-invasive ventilation (NIV) in acute excerbations can prevent the need for incubation, such that NIV should be available on a 24 hour basis in units managing patients with acute respiratory failure.
