ABSTRACT
The article discusses the medication huddle that Nationwide Children's Hospital (NCH) in Columbus, Ohio devised to launch a multidisciplinary procedure for both evaluating cases of adverse drug events (ADE) and determining solutions that will prevent each specific ADE from taking place again. It explains the steps taken to ease the huddle process including the promotion of an environment that encourages reporting, the development of a data collection tool, and the use of flexible scheduling.
