IMPACT OF MAJOR RISK FACTORS ON DEVELOPMENT OF ISCHEMIC CHANGES IN STENOTIC-OCCLUSIVE DISEASE IN PATIENTS WITH DIABETES MELLITUS TYPE 2

ABSTRACT
Diabetes mellitus is a major independent risk factor and powerful accelerator of atherosclerotic stenotic-occlusive disease. Stenotic-occlusive disease of the arteries of the lower extremities is one of the most common complications of atherosclerosis. The aim of this study was to determine the presence of multiple risk factors (MRF) and relationship between MRF score with the development of ischemic lesions in patients with atherosclerotic stenotic-occlusive disease of the lower extremities and diabetes mellitus type 2. We included a total of 146 patients, 102 males (69.9%) and 44 females (30.1%), range of 40-67 years. Following data were collected: medical history, physical exam, laboratory data and MRF score. We underwent Color Doppler exam of the lower extremity arteries, DSA angiography or/ and CT-angiography. Patients were divided into subgroups: macrovascular diabetic angiopathy and non-diabetic atherosclerotic disease. Fontaine stage III disease were present in 39% patients. In the study group (n=85), there were 8 patients with 2 MRF (10%), 28 patients with 3 MRF (33%), 28 patients with 4 MRF (33%), and 20 patients with 5 MRF (24%). Out of this patients (Fontaine stage III) we had 16 patients with 4 multiple risk factors. In the control group (n=61) the largest group was the group in Fontaine stage III, with 4 MRF. In the study group MRF score was =5.95, and in the control group MRF score was =4.05, p<0.05. Conclusion: The results suggest that the multiple risk factors in Fontaine stage III disease were the most common, and that patients in this stage of the disease were at high risk of cardiovascular events. In this study we found that in patients with type 2 diabetes mellitus, MRF score was significantly higher and there were larger number of patients in Fontaine stage III, i.e. ischemic stage of disease. That significantly affects the poor prognosis and makes overall treatment more difficult.
