ABSTRACT
The author argues that the refusal of some U.S. states to expand their Medicaid and set up health insurance exchanges may lead to revenue gap and clinical service losses among hospitals. Accordingly, the Supreme Court's 2012 decision on the constitutionality of the Patient Protection and Affordable Care Act gave states the right to refuse Medicaid expansion. The impact of states' decisions not to expand on safety-net hospitals is discussed. INSET: What About the Insurance Exchange.
