Introduction: In vitro fertilization (IVF) cycle cancellations may be related to poor response to gonadotropins, ovarian hyperstimulation syndrome (OHSS), etc. Our aim in this study was to evaluate why IVF cycles get cancelled, try to categorize the cancellations and propose approaches to prevent some of the cancellations. Material and Methods: 1848 patients were studied. Patients were divided into three groups: (1) IVF cycle cancellations during COH, (2) IVF cycle cancellations during OPU, (3) IVF cycle cancellations during ET. Every patient's age, period of infertility, etiology of infertility, selected gonadotropin protocol, total dosage administered along with time of cycle failure were recorded. Results: 69.7% of patients had IVF cycle cancellations during COH (Group 1). The most common indication was the poor response to gonadotropins (60%). 6.8% of patients had IVF cycle cancellations during OPU (Group 2). The most common indication was no sperm retrieval with 4.5%. 23.5% of patients had IVF cycle cancellations during embryo transfer (Group 3). Lack of fertilization with 16.5% was the most common indication in this group. Conclusion: Aging is an important factor in IVF cycle cancellations that cannot be modified. On the other hand, there are still other aspects that can be treated and/or prevented.
