The article offers pharmacy related updates. A survey by the American Society of Health-System Pharmacists (ASHP) reveals an increase in the percentage of hospitals with pharmacists providing consultation on specific patients between 2004 and 2011. Compliance with recommendation for the prevention and detection of controlled-substance diversion varies widely across hospitals. An experiential program for third-year students requires them to complete medication reconciliation for all patients.
