ABSTRACT
Systematic reviews provide the best guidance for clinical practice. The present systematic review addressed the question of what is the best methotrexate dosing strategy and route of administration in patients with rheumatoid arthritis in terms of optimizing clinical and radiological response while minimizing toxicity. It demonstrated that a start dose of 15 mg/week orally, escalating at 5 mg/month to 25-30 mg/week or the highest tolerable dose, yields the best results. A subsequent switch to subcutaneous administration in the case of an insufficient response may also be considered.
