Background: Benign metastasizing leiomyomata (BML) in the lungs is rarely encountered. It has been reported in patients with leiomyomas after myomectomy or hysterectomy. Case: A woman who presented for a physical examination and a chest X-ray to rule out tuberculosis demonstrated numerous pulmonary nodules, as confirmed by computed tomographic imaging. She is surgically menopausal after undergoing hysterectomy with bilateral salpingoophorectomy for a leiomyomatous uterus over 10 years ago. Wedge biopsies of the pulmonary nodules demonstrated BML, which were positive for estrogen and progesterone receptors. Conclusions: BMLs are a rare sequelae of uterine leiomyomata, the etiology of which has not been definitively elucidated. The diagnosis should be considered when a patient presents with multiple pulmonary nodules following or concurrently with hysterectomy or myomectomy for leiomyomas. (J GYNECOL SURG 25:95)
