The article focuses on a study which examined the risk between 20- and 25-gauge vitrectomy. The study examined 6,395 patients in Japan undergoing either 20- or 25-gauge surgery. It was found that 3,592 eyes in the 20-gauge cohort had developed endophthalmitis in one eye. Microbial contamination was observed in 5 of 85 irrigation fluid samples and one of 85 vitreous samples after 20-gauge vitrectomy.
