ABSTRACT
The article assesses a 48-year-old male patient's first computed tomography (CT) brain scan which revealed a right hyperdense middle cerebral artery sign (HMCAS). After seven hours, the patient developed new left-sided weakness and a second CT was performed. This showed an increase in the density of that middle cerebral artery (MCA) and early evidence of ischemia.
