ABSTRACT

Zusammenfassung  Die Wirbelsäulenchirurgie ist durch die Fortschritte auf dem Gebiet der Anästhesie und Radiologie in den letzten 20 Jahren stark beeinflusst worden. Dieser Aufschwung wurde auch durch die technische Entwicklung der Wirbelsäuleninstrumentationen gefördert, wobei insbesondere die Einführung von Pedikelschrauben und ventralen Abstützkörben (Cages) zu nennen ist. Beide Techniken ermöglichen Korrektur- und Stabilisierungsmethoden, die gerade die Tumorchirurgie sehr stark beeinflusst haben. Gleichzeitig haben diese technischen Fortschritte jedoch auch dazu beigetragen, dass die Tumorchirurgie auch von weniger erfahrenen Wirbelsäulenchirurgen durchgeführt wird, was die Ergebnisse negativ beeinflussen kann. Aus unserer Sicht sollte bei der Tumorchirurgie der Wirbelsäule gefordert werden, dass nur Kliniken, die eine bestimmte Anzahl von Tumoren pro Jahr operieren, diese Eingriffe auch durchführen dürfen. Insbesondere die En-bloc-Resektion, aber auch die intraläsional-marginale Resektion stellen hohe Anforderungen an das technische Geschick und Erfahrung des Operateurs, um möglichst optimale Ergebnisse zu erzielen. Eine Zweit- oder Drittoperation erschwert den Eingriff unnötigerweise, in der Regel ist dabei eine R0-Resektion nicht mehr zu erreichen. Deswegen erfordert die Tumorchirurgie der Wirbelsäule ein standardisiertes Gesamtkonzept, das jedoch auch an die individuellen Probleme angepasst werden muss. Dies wird am besten im Rahmen eines Tumor-Boards festgelegt, wobei auch hier der Einsatz und die Möglichkeiten sowie der Zeitpunkt der adjuvanten Therapie exakt festgelegt werden. Nur eine solche „konzertierte Aktion“ ermöglicht es, mittel- und langfristig gute Ergebnisse zu erzielen.
