ABSTRACT
The introduction of new COX-2-specific nonsteroidal anti-inflammatory drugs (NSAIDs) has underscored the complexity of drug selection within the NSAID class. While NSAIDs in general are essentially equivalent in efficacy for their major indications, they may differ in many other factors that influence formulary selection and appropriateness for any given patient. This article provides a general overview of those factors and presents recommendations, based on this overview, for a formulary approach to the NSAID class. Beneficial attributes that should be weighted heavily in formulary assessments of NSAIDs include generic availability, relatively low acquisition cost, multiple dosage forms, approval for an array of indications or for a unique indication, a low incidence of gastrointestinal bleeding, and a convenient dosing regimen.
