ABSTRACT
The article focuses on a research at the Mayo Clinic College of Medicine in Rochester, Minnesota about the impact of the use of a multidisciplinary medication reconciliation process on the rate of medication discrepancies during patient admission and discharge. It relates the role of lack of medication reconciliation in medication errors and adverse drug reactions. It describes the medical reconciliation program established at the Mayo Clinic. The study has found that discrepancies occurred more commonly during discharge than during admission.
