ABSTRACT
The article focuses on the Interim Care Program (ICP), a four-hospital partnership with the county of Sacramento in California, the Salvation Army and The Effort, a community-based primary-care and mental health provider, which provides beds and follow-up medical care to homeless patients. The four participating hospitals, including Sutter Medical Center, committed $50,000 a year to the ICP as well as beds and equipment. The start of pilot testing of the phase II of the ICP, the T3 Continuum Program, was in early 2007.
