ABSTRACT
The article discusses the efforts of leaders at Advocate Health Care to create a culture of safety that permeates every level of the organization. By the late 1990s, Advocate's board of trustees identified patient safety as a system wide priority. A task force was established to explore ways to improve performance. As Advocate pressed forward with its efforts, leaders shared a feeling that something was missing. Safety was always a priority, says Leonard Kosova, a hematologist and oncologist who has worked at Advocate's Lutheran General Hospital.
