ABSTRACT
The article focuses on problems related to patient safety and steps taken to ensure it. In the field of medicine, a high error rate is unacceptable but researches show that one of every five medication doses administered in hospitals in the United States may be erratic. Such errors along with death create patient distrust. The impediments to patient safety are mostly human. It is necessary to appoint a medication safety officer and maintain a national database for ensuring patient safety.
