The 131 I fibrinogen test is not only a useful and simple test for the early detection of occult forming thrombosis, but also a satisfactorily accurate method for confirming the clinical diagnosis of established deep vein thrombosis. This was shown by three groups of experiments in a total of 259 patients: 1) A complete correlation with ascending phlebography was found in 39/47 patients (=83%). Using 131 I fibrinogen with its higher tissue penetration compared with 125 I fibrinogen also thromboses in the deep thigh veins could be detected. 2) In three cases with superficial thrombophlehitis taken as a model for deep vein thrombosis excisions of thrombosed veins were made 24 hours after 131 I fibrinogen injection. In vitro measurements of radioactivity revealed more fibrinogen/fibrin in the vein wall and in the surrounding fatty tissue than in the thrombus itself. This shows that the substrate for a positive fibrinogen test in established thrombosis is not only due to the active intravascular clotting but also to an accumulation of fibrinogen in the vein wall and the surrounding fatty tissue. 3) 22 patients undergoing sclerotherapy of their varicose veins were taken as a model for artificial thromhosis with a distinct time of onset. It was shown that the fibrinogen kinetics in forming and established thrombosis were different thus confirming different substrates for a positive test in such cases.
