In 12 patients with an acute deep venous thrombosis in one leg and the contralateral normal, both legs have been compared plethysmographcally during external pressure and elevation. The factors studied were the increase in the volume of the leg at venous stasis (venous volume) and the speed of volume decrease after the release of venous stasis (venous emptying). In the calf and ankle of the normal leg, external pressure as well as elevation increased the venous volume and venous emptying, due to better emptying of the veins before measurement; in the thrombotic calf, neither external pressure nor elevation increased the venous volume or venous emptying. The thrombotic ankle showed a greater venous volume but unchanged venous emptying. The reduced venous emptying seems to he caused by the space occupying effect of the thrombosjs decreasing the venous volume and increasing the flow resistance of the veins.
