ABSTRACT
The article presents an answer to the question about the diagnosis of a peripheral blood smear and an abdominal T2 weighed magnetic resonance image of an elderly lady. The peripheral blood smear revealed numerous deformed red blood cells with thorny projections. Magnetic resonance imaging demonstrated diffuse hypointensity of the liver which suggests heavy iron loading. Spur cell anemia of alcoholic cirrhosis was diagnosed. It is characterized by an increased ratio of free cholesterol to phospholipid in the erythrocytic membranes that results in acanthocytes.
