Spontaneous intracranial hypotension (SIH) is a neurologic disorder that typtcally presents with orthostatic headaches due to low cerebrospinal fluid (CSF) pressure and/or CSF leakage, usually accompanied by neck pain, tinnitus, disturbed hearing, photophobia and/or nausea. SIH symptoms disappear after correction of CSF pressure. Diagnosis of spontaneous intracranial hypotension, despite extensive investigation, remains a significant challenge in daily clinical practice due to heterogeneous presentation. It is estimated that the number of undiagnosed cases may be as high as 94%. Underdiagnosis and incorrect treatment of SIH negatively impact patients' quality of life and cause negative economic and social consequences. This article summarises recent data about SIH and presents a clinical case.
