ABSTRACT
High ceiling diuretics are the cornerstone in the treatment of patients with congestive heart failure and cardiorenal syndrome. Loop diuretics are recommended by the ACCF and AHA as initial treatment, during acute exacerbation of congestive heart failure and also in non-hospitalized patients showing congestive symptoms. However, some controversies exist in relation to the appropriate dosis, and its combination with thiazides and aldosterone antagonists. Those facts are related to high variability in pharmacokinetics and comorbidities presented by patients with this pathology; such as advanced age, caquexia, hypoalbuminemia and reduced glomerular filtration rate, just to mention the more important involved factors. Urinary sodium and diuretic excretion should be periodically evaluated in order to estimate natriuresis. NSAID should be omitted. Current guidelines recommend patient education about proper management of high ceiling diuretics administered through the oral route.
